This Award recognizes outstanding personal achievement in the field of Hospice Palliative Care in British Columbia, in either a paid or volunteer position.
Nominees must have been involved in Hospice Palliative Care for a minimum of five years and have displayed excellence and achievement in one or more of the following fields: clinical practice, education, research, advocacy, or resource development. Their accomplishments should have positively affected the quality of life for patients and families facing death, dying, and bereavement and/or have enhanced the profile of Hospice Palliative Care in British Columbia.
The nominee’s service should have:
- demonstrated leadership and commitment through outstanding achievement;
- made a notable impact by significantly strengthening the Hospice Palliative Care program, the lives of patients and families served, and/or the community;
- enhanced the profile of Hospice Palliative Care in the community, British Columbia, Canada, or internationally.
The Award is presented each year to one individual who meets the above criteria.
Please submit to BCHPCA the nomination form on the next page and attach two (2) letters of support which demonstrate the nominee’s skills, talents, and accomplishments related to this award.
